Outpatient trial investigating medications that add longevity in heart failure - optimal-HF study

Outpatients with heart failure of any cause (with an ejection fraction less than or equal to 40%)are eligible.

This study will test the effectiveness of a no-cost consultation by a heart failure nurse specialist (that includes a medication review) to assist patients with heart failure learn about the optimum medications and therapies to treat their heart failure.

Participants are have an initial 2 hour consultation with 15-minute phone calls one and three months out.

No compensation provided for this study.
